I understand that playing tennis involves risks of injury. I agree that Eisabeygi Tennis Club, its coaches, staff, and volunteers are not responsible for any injury, accident, illness, or property loss during participation. I take full responsibility for myself/my child.

In case of emergency, I allow club staff/volunteers to seek medical treatment for myself/my child. I accept all costs related to such treatment.
